First was the Limbaugh smear, orchestrated by the Soros-influenced (and likely Soros financed) Media Matters propaganda outlet.
Second was the purposeful misrepresentation of the O'Reilly conversation with Juan Williams, which has been misrepresented on local blogs as well as on national media outlets.
Peter Wehner exposes the lies about Limbaugh.

Regarding the smears of O'Reilly, here's an spot-on analysis of the structuring of the attack.
